For First Time Appointments/Evaluations: 

If you have an evaluation scheduled with one of our psychologists, please complete the forms below. These forms are necessary to provide you with office guidelines and to obtain preliminary background information.After you complete the forms you can either email them to your psychologist or bring them to your scheduled evaluation. If you are unable to complete them beforehand, you are asked to come in a few minutes early to the appointment.
